Required for All Chorus Families 

Parents/Guardians of all RETURNING & NEW chorus members are required to

complete the online forms linked below.

Absence Request Form

  1. Known absences should be reported to the Chorus staff at least 2 weeks in advance. 

  2. Please use this form, linked below, to also submit requests to arrive late and leave rehearsal early.

  3. Angel will contact the chorister and their family only if the excused absence request was denied.
